The Heart of Sorrow is a post-Sisters of Discord tier Expert Mode-exclusive accessory that is obtained from the Treasure Bag dropped by Sisters of Discord. While equipped, your melee and ranged attacks become stronger the less health you have, your melee and ranged attacks inflict Hydratoxin when above 1/3 of your maximum life and Moonraze when below that health threshold. Also, your movement speed is doubled when below 2/3 of your maximum life.
Its Inferno counterpart is Heart of Passion, which focuses on mage and summoner users.
